FreeMedForms is free and open source software, available for all current platforms (Linux, Mac OS X, Windows and FreeBSD). It is powerful and fully configurable. It can be adapted to many needs: various medical specialties, para-medical specialties and even veterinary profession.

The patient file is fully editable. It is built with an XML file that you can modify to suit best for your practice. You can also customize your main patient file by adding subforms. FreeMedForms comes with an extensive list of subforms.

With FreeMedForms you can set your personal agenda. Create an appointment for one or more patients in a single click ! With the MySQL technology, you can delegate the agenda management to a distant secretariat.

With its PMHx component, FreeMedForms prepares the patient file for automatic computations such as the identification of potentially inappropriate medications in the elderly (still experimental). PMHx can be both forms (such as clickable risk factors) or traditional items.

FreeMedForms has a prescription module. Several databases treatment are available: France, Canada, the United States of America and Belgium. This module can detect drug-drug interactions as well as patient-drug interactions. Its data are open and verifiable (distributed under GPLv3).

FreeMedForms is fully multi-user. You can set up multiple user profiles such as: doctors, nurses, caretakers, secretaries… An ergonomic interface helps you manage your users.
